Oral Appliances & Airway-Focused Orthodontics
Day and/or night solutions to open the airway, calm the jaw, and correct the root cause—guided by data.
Why we use oral appliances
A healthy airway needs enough room for the tongue, a well-balanced bite, and nasal breathing. Oral appliances help us create that space, guide growth, relax overworked jaw muscles, and stabilize the airway.
Important: These aren’t “night guards.” Many of our appliances are worn day and/or night (by schedule) to change structure; others are worn at night to relieve symptoms. We’ll match the approach to your anatomy and goals—and re-test to prove progress.
Root-cause options (structure first)
Guided growth for children
Goal: Grow the jaws wide/forward so the tongue fits up in the palate, teeth have room, and sleep is more restful.
Wear: Short, consistent day and/or night wear as directed.
Good for: Mouth-breathing, early crowding, narrow arches, snoring/restless sleep, bedwetting, focus/behavior concerns.
Palatal/arch expansion for teens & adults
Goal: Widen a narrow palate/arch to increase tongue space and support nasal airflow; often improves bite and facial balance.
Wear: Day and/or night depending on device; periodic checks.
Good for: High/narrow palate, dental crowding/relapse after braces, mouth-breathing, airway impingement.
Clear aligners planned for airway & bite
Goal: Straighten teeth while protecting or gaining arch form and tongue room (not just “straight teeth”).
Wear: Typically 20–22 hours/day with weekly or biweekly changes.
Good for: Alignment and bite improvement when we also want long-term airway support.
Night-focused sleep appliances (symptom relief)
Mandibular Advancement Devices (MADs) — Snoring front-and-center
What they do: Gently position the lower jaw forward to stabilize the airway during sleep.
Snoring benefits: Often quieter nights right away, fewer awakenings, and happier bed partners. We titrate in small steps and re-test to confirm real change.
Best for: Primary snoring, and many mild–moderate sleep-disordered breathing/OSA cases; great for CPAP-intolerant patients and travel.
How we do it: Custom fit → comfortable, stepwise titration guided by symptoms and data → home sleep testing to verify improvements in snoring intensity, AHI/RDI, oxygen trends, and sleep quality.
Combine when needed: Many patients start with a MAD for fast snoring relief, then add expansion/aligners to address structure for long-term stability.
Orthotics & vertical support (when damage is present)
Clenching, grinding, or a collapsed bite can strain joints and muscles. Orthotic therapy restores a healthier vertical dimension and jaw position so muscles can relax and teeth are protected—often a bridge to stable expansion/aligners or restorative care.
Wear: Day and/or night depending on diagnosis; closely monitored.
What to expect with us
- Airway-focused exam – nasal airflow, tongue posture/space, palate & jaw width, bite and muscle comfort.
- Records – digital scans and photos; CBCT review when appropriate (visualize airway shape/volume and joints).
- Plan – we partner with multiple labs and choose the device that fits your anatomy and goals.
- Fit & coaching – clear wear schedule (day vs night), hygiene tips, comfort checks.
- Adjust/titrate – small changes guided by symptoms and objective data.
- Re-test – home sleep testing read by a board-certified sleep physician to verify improvement.
- Maintain – periodic visits to protect results and long-term bite stability.
Who’s a good candidate?
- Kids with mouth-breathing, early crowding, snoring, or restless sleep
- Teens/adults with a high or narrow palate, tongue crowding, or relapse after braces
- Snoring or mild–moderate OSA, CPAP-intolerant, or frequent travelers
- TMJ pain, clenching/grinding, bite wear, or loss of vertical support
What results look like
- Quieter nights and happier bed partners; fewer awakenings
- Better daytime energy and focus
- AHI/RDI, oxygen trends, and sleep quality that improve on re-tests
- Calmer jaw muscles, fewer headaches, a bite that feels stable
- Measurable arch/tongue-space changes when we expand
FAQs
Are these just “night guards”?
No. Many are day and/or night appliances to change structure; others are night-focused to relieve snoring and apnea symptoms.
Will aligners collapse my arch?
Ours are planned to support or gain arch form and tongue space while improving the bite.
MAD vs. expansion—how do I pick?
MADs reduce snoring quickly for many people at night. Expansion addresses structure (tongue space/nasal airflow). Many patients do both, in stages, with testing to guide timing.
Will my bite change?
Temporary shifts can occur during active care. We monitor and adjust to protect joints and achieve stable results.
Is this coordinated with medicine?
Yes. Home sleep studies are read by a board-certified sleep physician, and we share results with your medical team when appropriate.
